On this link to Rainbow Bars, there is a swirled design cookie in the top frame...

I think it may be done in two stages:

Caramel color is done by laying down stripes of caramel icing (on the white background). Before it hardenes, start to the left of the rows and drag a sharp knife--perpendicular to the row--to the right across the entire surface. The icing will drag to a point. Now move down half an inch, but drag the knife to the left in the opposite direction. The icing will drag in the opposite way. You'll end up with alternate serifs/swirls.

If you're not comfortable dragging the opposite direction, drag in one direction, moving down every inch until you get to the bottom. Then flip the cookie/cake around 180 degrees and drag between the lines you've already done. Visually, you'll get alternating serifs/swirls.

After that hardens, put dots of dark chocolate icing wherever and with a toothpick, start above the dot and drag through it, pulling down. This makes a heart. She also put caramel hearts on too.

She's had a lot of practice to make it look this smooth.

 
M is right--you drag one way and then the other way--two toned heart--two dots (milk & dark)..

 
Same procedure as that used on Napoleons--a heart is made simply by dragging a toothpick thru a dot

 
I've done spider webs and things, so I'm familiar with the technique... for some reason

I was just thinking this was all done at once and my mind froze!
